I do, hereby, release The Town of Lake Waccamaw (herein referred to as TLW) and it's directors, officers, employees and agents from any and all claims, causes of action or liability, which I now have or which may arise in the future resulting from or arising from my association with TLW. I further, hereby, waive any and all claims, which may arise in consideration of the waiver of claims by other volunteers. I acknowledge that I have read the foregoing Waiver and Release Form and have executed the same freely and voluntarily. I further affirm that I am at least 18 years old (or have parent / guardian consent to participate) not currently the subject of any criminal investigation or prosecution, nor do I have a criminal record in the State of Florida or any other locale.
